Unique failure?

How widespread are the failures within the Probation service highlighted by the murder of financier John Monckton?

Had he not been murdered, how would the failures have been highlighted? Will other cases handled by the four suspended probation officers now be reviewed urgently? Their lack of diligence is unlikely to be unique to this case.

Would Damien Hanson's attendance at an anger management course really have prevented the murder?
